This information is for Parents/Guardians of Grade 7 students only

 

Your child will be coming home today with a package of information regarding the School Immunization Program. This package will include a form called Consent and the School Immunization Program (white paper), as well as a Hepatitus B Form (yellow heading), Human Papillomavirus (purple heading), and Meningococcal (orange heading). Please read the information carefully and return the bottom portion of each of the 3 forms with your child no later than Monday, September 12th.
